The creation of the Wellness Center concept was born out of the genuine concern of team members at Regency to address the ongoing health concerns of residents in Senior Independent Living Communities throughout Chicagoland. The goal of this program is to provide on-site care, services and on-going education to residents who have limited or episodic need for assistance and information; have not been assigned to home health care services; are without a primary care physician; and or refuse ‘assistance’ for fear of a loss in appropriateness for independent living. It is our sincere hope that by providing assistance in early stages and care when needed, we can indeed assist our participants to maintain their independence and more importantly their health for as long as possible.
Through strategic partnerships with Diagnostics, Home Medical Equipment and Pharmacy and most importantly, this Senior Living Community, we are able to offer the following services*:

| 2 hrs | 2 x week | 10 – 19 participants | |
| 2 hrs | 3 x week | 20 – 29 participants | |
| **schedule for more than 30 participants will be established at the time of need. | |||
| Blood pressure checks, blood sugar checks, medication sorting, need assessment | |||
| Physician | 1 x month | 1 – 15 patients |
| 2 x month | 16 – 30 patients | |
| Wound assessment, screenings, check ups, allergy testing, x-rays, assessment for home health, equipment needs and specialist referrals, medication managemen | ||
| Pharmacy | 1 x month |
| Medication and supply delivery as needed | |
| Medicine Cabinet Cleaning, checking for prescription drug and OTC interactions, small DME, disposables i.e. Depends, etc. |
| Equipment | 1 x month | Quarterly depending on need after first visit |
| Assessment of existing equipment (replacement if needed), demonstrations on proper use, cleaning and maintenance of purchased (NOT rented) equipment, suggestions for participants on useful tools including but not limited to walkers, rollators, canes, raised toilet seats, bed side commodes. | ||
We will also bring in representatives and information from the Department on Aging, The American Heart Association, American Cancer Society, Alzheimer’s Association, American Diabetes Association and the Arthritis Foundation.
* Every effort will be made to adhere to the general outline above. They may be occasions when services offered may need to be rescheduled or limited based on staff availability. There may also be instances when an increase in services is available based on need.
